Inmate Fax Cover Sheet (needed for ALL inmate requests, i.e. vehicle releases/marriages/everything)
This is the form that is required if you are unable to appear for your marriage at the County Clerk's office. This could be due to incarceration or hospitalization, or another reason approved by the County Clerk. We take this form in with us to our signings and do not ask our customers to provide an extra copy to us.
If you would like to request a notary signing for an inmate, please download this cover sheet and send it along with your documents so we have all of the information we need to make your signing a smooth process. Once we receive your documents and book an appointment, a notary will call you to go over the delivery of your documents and payment options. Please download this document and customize it for your specific case. We cannot transcribe these forms for you but are providing an example of what a statement should look like. Many customers like to also include the "release of personal property" along with the "vehicle release" to ensure all of the inmate's personal property is picked up.
Inmate Wedding Order Form (2013)
If you would like to order inmate wedding services from us, please fill out and send us the PDF above. This form is needed even if you only need the marriage license notarized (check inability to appear only). You will find our wedding officiant's information at the bottom of the page which you will need to give to the jail. For a list of instructions regarding the inmate wedding process, please visit our inmate wedding page. If you have questions regarding the wedding process after reading our inmate wedding page, please call the Alameda County Clerk's office (they handle all legal questions). If you have specific questions in regards to your marriage, please email questions to email@example.com (please list your questions in numerical order). We no longer take calls which ask "how do I get married to an inmate?" This information is provided on our blog and website and is information that should be provided to you by the Alameda County Clerk (please call them before you call us!). We simply take care of the entire wedding for you, and dont discuss how we process it until after you submit a wedding order form to us. Currently weddings are taking about 3 months, and sometimes they can be pushed up earlier. We do not perform weddings during visiting as this violates the jail's rules.

Issuance of Passport to Minor Form
If one of the parents or legal guardians cannot be present we use the Statement of Consent Regarding a Minor(DS-3053) form to obtain the US Passport. Since we have this form, there is no need to fax or email us another copy. We do need you to send us an Inmate Notary Order Form and under "comments" please let the notary to take in the passport form and provide the child's full legal name and birthdate.
